CSU Minor Media Release Form

The California State University (CSU) Minor Media Release form grants San José State University, along with the CSU, permission to use photography, video, and/or audio that significantly aids in showcasing the vibrancy and diversity of our university community.

If you are under 18 years old, a parent or legal guardian will need to fill out the form on your behalf. You may also download the Minor CSU Release Form.
