CSU Media Release Forms

The California State University (CSU) Media Release form grants San José State University, along with the CSU, permission to use photography, video, and/or audio that significantly aids in showcasing the vibrancy and diversity of our university community.

Please fill out the following Adult form If you are 18 years of age or older. You may also download the Adult CSU Release Form.

If you are under 18 years old, a parent or legal guardian will need to fill out the minor release form on your behalf.

By signing this form, you acknowledge that you're providing your consent without any expectation of compensation.
