Key and Building Card Access Requests

Procedure

1. Email the department administrators at nutrition@sjsu.edu to request the Key and Card Request Form, which is completed via Docusign.
2. After the request form has been signed, forwarded to Facilities, and Facilities has completed your request, they will send you an email informing you that your keys or card access is ready.

• What do I do if I have not received an email from Facilities saying I have access and I submitted my form a while ago.

• What do I do if Facilities emailed saying I should have card access, but I don’t?
