  3. WELLNESS INVENTORY ASSIGNMENT 
    1. Go to http://wellness.uwsp.edu/Health_Service/Services/livewell/  (click on highlighted text)
    2. Complete the questionnaire 
    3. Do not fill in the name field at the bottom of the questionnaire. Instead, use the following 8 character code:

      Male or female (m/f): _____
      First 3 letters of mother's maiden name: _____  _____   _____
      First two letters of the month you were born: _____  _____
      Last two digits of your zip code: _____  _____

      For example, Dr. G's code is mBarMa03 because he is male, his mother's maiden name is Barnett, he was born in May, and his zip code ends in 03.

    4. Click the "Evaluate" button.
    5. Print two copies of the evaluation summary. Retain one copy. Hand the other copy in next Tu (2/10) -- worth 1 "quiz" point -- late assignments not accepted. 
    6. Click here for an example of the printout. Note the various categories. Note what scores mean.

    3. Students will review each other's presentations ("Peer Reviews") on day of the presentation ( will be discussed at future date).
